How Tooth Decay Develops
Tooth decay begins when bacteria in plaque metabolise sugars and starches from the diet, producing acids as a by-product. These acids gradually dissolve the mineral content of the tooth enamel in a process called demineralisation. Initially this is reversible if caught early, but with repeated acid exposure and inadequate cleaning, the enamel eventually breaks down, forming a cavity, which is a permanent hole in the tooth structure.
Once decay penetrates through the enamel into the underlying dentine, it tends to progress more rapidly because dentine is softer and less resistant to acid. If left untreated, decay can eventually reach the pulp, the nerve and blood vessel centre of the tooth, causing significant pain and potentially requiring root canal treatment.

Early decay often produces no symptoms at all, which is why cavities are frequently discovered during routine dental examinations or X-rays before the patient notices anything. As decay progresses, sensitivity to sweet, hot or cold foods may develop, followed by visible dark spots or holes, and eventually spontaneous pain, particularly when the decay approaches the nerve.
Because significant decay can exist without obvious symptoms, especially between teeth or beneath old fillings, relying solely on the absence of pain to judge dental health can allow problems to progress unnoticed. Regular check-ups including X-rays when appropriate remain the most reliable way to catch decay early.

Frequent consumption of sugary snacks, sweetened beverages and traditional sweets, particularly between meals rather than with meals, significantly increases decay risk by providing repeated acid attacks throughout the day. Inconsistent brushing, particularly skipping night-time brushing, allows these acids to act on teeth for extended periods overnight.
Reduced saliva flow from certain medications or dehydration, insufficient fluoride exposure in some regions, and limited access to regular dental care in certain areas can also contribute to higher decay rates. Treatment Options by Severity
Very early decay confined to the enamel surface can sometimes be managed with fluoride treatments and improved oral hygiene, encouraging remineralisation without the need for a filling. Once a cavity has formed, the decayed portion of the tooth is removed and the space restored with a filling material, most commonly tooth-coloured composite resin, to restore both function and appearance.
More extensive decay may require a crown to protect a weakened tooth structure, while decay reaching the nerve typically requires root canal treatment followed by a crown. Prompt treatment at the earliest possible stage generally means simpler, less costly and more conservative options, underscoring the value of regular dental visits.
