Dental emergencies do not wait. Severe throbbing pain, facial swelling, a knocked-out tooth or trauma after an accident all have a time window in which the outcome is materially better.
As a hospital with an oral and maxillofacial surgeon on the panel, we manage facial swellings, trauma and infections that a general clinic would refer onward.
Facial swelling is the one that matters most
A dental infection that has spread into the facial spaces can compromise the airway and swallowing. Swelling that is spreading toward the eye or under the jaw, difficulty swallowing, fever, or difficulty opening the mouth are reasons to be seen immediately.
Antibiotics alone do not resolve a dental abscess. The source has to be drained or treated, otherwise the infection returns as soon as the course finishes.
A knocked-out permanent tooth
Pick it up by the crown, never the root. If it is dirty, rinse it briefly in milk or saline — not water, and do not scrub it. Re-implant it into the socket immediately if you can, and bite gently on a clean cloth.
If you cannot re-implant it, store it in milk or in the patient's own saliva and get to us within the hour. Dry storage destroys the ligament cells and with them the chance of saving the tooth.
Pain relief until you arrive
Take the analgesic you normally tolerate at the correct dose, apply a cold compress externally for swelling, and keep your head elevated when lying down.
Do not place aspirin against the gum — it causes a chemical burn — and do not apply heat to a facial swelling.

Should I just take antibiotics?
Antibiotics buy time in a spreading infection but never cure a dental abscess. The tooth still needs treating, and delaying that is how a small problem becomes an admission.
My child knocked out a baby tooth. Should I put it back?
No. A knocked-out milk tooth should not be re-implanted, because doing so can damage the permanent tooth developing beneath it. Bring the child in for assessment.
