The Demineralisation and Remineralisation Cycle

Throughout the day, teeth undergo a natural cycle of demineralisation, where acids from bacteria and food weaken the enamel surface, and remineralisation, where minerals from saliva redeposit into the enamel to repair this early damage. When demineralisation outpaces remineralisation, due to frequent sugar exposure or inadequate oral hygiene, decay eventually results.

Fluoride assists this natural repair process by being incorporated into the enamel structure, forming a more acid-resistant mineral that is better able to withstand future acid attacks. This is why fluoride is considered one of the most effective tools available for preventing tooth decay across all age groups.

Sources of Fluoride

Fluoride toothpaste used twice daily is the most accessible and important source of fluoride for most people, delivering a small, safe, topical dose directly to the tooth surface with every brushing session. Some municipal water supplies also contain fluoride, though levels vary considerably across different regions in India.

For patients at higher risk of decay, professionally applied fluoride varnish or gel provides a more concentrated, targeted dose during dental visits. This is particularly beneficial for children, patients with a history of frequent cavities, and those with reduced saliva flow that limits natural remineralisation.

Using Fluoride Safely and Effectively

For most adults and children over the recommended age, brushing twice daily with a fluoride toothpaste is sufficient for good protection, using an appropriately sized amount for the child's age to avoid excessive swallowing during the early years of tooth brushing. Spitting out rather than rinsing thoroughly after brushing allows fluoride to remain on the teeth slightly longer for continued benefit.

Excessive fluoride intake during childhood tooth development, typically from swallowing large amounts of toothpaste repeatedly, can occasionally cause mild white marks on adult teeth, known as fluorosis, which is why supervising young children during brushing and using only a pea-sized or smaller amount is recommended.

Yes, fluoride toothpaste is safe and beneficial for children when used in the appropriate amount for their age, typically a small smear for very young children and a pea-sized amount for older children, with adult supervision to minimise swallowing.
Can fluoride reverse an existing cavity?
Fluoride can help reverse very early decay that has not yet progressed to a true cavity with structural breakdown, by supporting remineralisation of weakened enamel. Once an actual cavity has formed, a filling is required as the tooth structure cannot regenerate.
Do I need professional fluoride treatment if I already use fluoride toothpaste?
Most people with low decay risk do not require additional professional fluoride treatment beyond regular toothpaste use. However, patients at higher risk of decay, including many children, may benefit from the more concentrated protection professional fluoride application provides.
Is too much fluoride harmful?
Fluoride is safe within recommended amounts, but consistently excessive intake, particularly by young children swallowing large amounts of toothpaste, can cause cosmetic changes to developing adult teeth called fluorosis. Using the correct amount for age and supervising brushing helps avoid this.
