Wisdom teeth cause trouble when there is not enough room for them to erupt straight. They push against the second molar, trap food and bacteria under a flap of gum, and cause repeated episodes of swelling and pain that antibiotics settle only temporarily.
Every impacted third molar here is assessed against the inferior alveolar nerve before removal. Where the roots sit close to the nerve on an OPG, we take a CBCT scan rather than proceed on a two-dimensional guess.
Does every wisdom tooth need to come out?
No — and we will tell you when it does not. A fully erupted, cleanable, symptom-free wisdom tooth in a healthy bite can be left alone and monitored.
Removal is indicated where there is repeated pericoronitis, decay in the wisdom tooth or the molar in front of it, a cyst on radiograph, damage to the adjacent root, or where the tooth is obstructing orthodontic treatment.
The nerve question
The inferior alveolar nerve supplies sensation to the lower lip and chin, and it runs close to the roots of lower wisdom teeth. Temporary or permanent altered sensation is the complication patients fear, and rightly so.
Where an OPG suggests the roots are in contact with the canal, a CBCT scan shows the true relationship in three dimensions. In high-risk cases a coronectomy — removing the crown and deliberately leaving the roots undisturbed — is often the safer choice, and we will recommend it where it applies.
Recovery, honestly described
Swelling peaks at 48 to 72 hours and then settles. Expect limited mouth opening for a few days and a sore jaw for about a week. Most patients take two days off work for a difficult lower impaction and none for a simple upper one.
Dry socket, the most common complication, is largely preventable: no smoking, no rinsing on day one, no straws. We review you afterwards rather than sending you away with a leaflet.
