Extraction is the last option, not the first. Before removing any tooth we assess honestly whether root canal treatment, a crown, or periodontal therapy could keep it in function for another decade — and we say so when it could.
When a tooth genuinely cannot be saved, how it is removed matters. Atraumatic technique preserves the surrounding bone, which is what makes the eventual implant simpler and cheaper.
When a tooth cannot be saved
Vertical root fracture, decay extending well below the gum and bone level, advanced periodontal disease with severe mobility, and failed root canal treatment where re-treatment is not viable are the common reasons.
Teeth are also removed for orthodontic reasons where the arch is severely crowded, and occasionally before radiotherapy or major medical treatment on the advice of the treating physician.
Plan the gap before you create it
The worst outcome of an extraction is an unplanned gap. Neighbouring teeth tilt into it, the opposing tooth over-erupts, and a straightforward single implant becomes a complicated multi-tooth problem three years later.

Frequently asked questions
Will it hurt?
The extraction itself is done under local anaesthesia and is not painful. Discomfort for a day or two afterwards is normal and managed with simple painkillers.
How long before I can eat normally?
Soft food the same day, near-normal eating within three to four days for a simple extraction, and about a week for a surgical one.
How soon can the tooth be replaced?
An implant can sometimes be placed immediately, otherwise after eight to twelve weeks of healing. A temporary tooth can be provided in the interim if the gap is visible.
