Orthognathic surgery repositions the upper jaw, the lower jaw, or both, to correct a skeletal discrepancy. It is the treatment for a severe underbite, a marked open bite, significant facial asymmetry or a receded lower jaw that braces alone cannot address.
It is combined treatment: an orthodontist aligns the teeth within each jaw first, and the surgeon then places the jaws in the correct relationship to each other. Planning is done jointly from the outset.
Who needs it
Patients whose bite cannot be corrected by moving teeth alone — a Class III underbite where the lower jaw is genuinely longer, an anterior open bite that persists after growth, chin asymmetry, or a severely receded lower jaw with airway consequences.
The diagnosis is skeletal, not dental. Cephalometric analysis and a CBCT scan determine whether the problem lies in the teeth or in the bones carrying them.
The treatment timeline
Pre-surgical orthodontics typically takes twelve to eighteen months. Surgery follows, then a further six to nine months of finishing orthodontics. It is a long pathway and we set that expectation clearly at the first consultation.
Surgery is performed under general anaesthesia with fixation plates, so the jaws are not usually wired shut afterwards. Most patients are on a soft diet for six weeks.
